Youth Grant Application Guidelines to EAYA Secretariat.


Please type, circle and tick within the boxes in your form. Please make a photocopy of the form before you try and fill the original coipis. Attach only requested documents, and add appendices when absolutely necessary.

1. What is your proposed area of the project ?

PERSONAL DETAILS

1.1. What is the name of the group/ organisation?

1.1.1. Give full details concerning the organisations address, Telephone and fax, name of your bank and account number.

1.1.2. What country in East Africa does your group operate in?

1.1.3. What is the scope of operation of the group/ organisation?

1.2. How long has it been in operation?

1.3. Give names of 3 top officials and their qualifications?

1.3.1. Please attach a passport photograph copy for each of the three next to their names.

1.4. Is your group registered with the government of your country?

1.5. How many members are registered with your group?

1.6. Has your organisation been involved in projects before? Please state type of project, area of project, funding agency (if any) and amount of money used in the project.


PROJECT DETAILS

2.0. Give your project title which should clearly describe the objectives of the proposed project.

2.1. Give a short summary (about 1000 words) of the proposed project, this should be precise, succinct and efficiently informative.

2.1.1. State the methods you intend to use in the project.

2.2. Provide information about results you have achieved in the area of the proposed project.

2.3. Mention any past, future or planned collaborative projects with other groups or organisations.

2.4. Give objectives of the project. These should describe the problem and how you propose to solve it. Distinguish between long term and short term objectives. How will the short term objectives solve the long term objectives.

2.5. Give the wokplan or the description of the project as the main item of the proposal. Remember each proposal is assessed by the secretariat team and later by the granting institution. You need therefore to convince the institution that your proposal is the best.

3.1. Cost of Materials

3.2. Cost of stationery for reporting.

3.3. Cost of consultancy (If any)

3.4. Cost of per-diem allowance for key project personell

3.5. Budget justification.


CONTACTS

4.1. Provide names, titles, positions and full addresses of three referees with whom further information may be sought.

4.2. If the organisation/ group has a patron, s/he should endorse the application.


NOTE:

ALL PROSALS SHOLD BE TYPED AND CLEAR.

SEND TWO COPIES OF THESE FORMS.

KEEP A COPY FOR YOUR FURTHER USE INCASE WE COTACTED YOU FOR CHANGES IN THE PROPOSAL.

BE SURE TO STATE IN A LETTER OF INTRODUCTION THE AREA YOU THINK YOUR PROJECT FALLS IN OUR 6 AREAS OF OPERATION.

ALL PASSPORT PHOTOS SHOULD BE SIGNED AT THE BACK BY THEIR OWNERS.
